How To Complete The Gym Clinic Application Form:

1. Download the application form here
2. Only use Capital Letters
3. If you are an adult (over the age of 18 years) please complete Section A, C and D.
4. If you're taking any contract package, debit order details are compulsory - please complete Section D.
5. If you're taking a contract package, please provide us with your ID and Proof of Residence. Proof of residence should be in your name and less than 4 months old.
6. Upload the completed form using the upload button below

Once completed, immediately go to the gym to complete your registration.

For Debit Orders: Please do have your Cell Phone with you for DebiCheck. Preferably, you will have to have a Banking App to approve the Debit Order.

If you do not have a Banking App, still have your phone with you and your cell phone number should be linked to your Bank Account.
